Most evaluations are submitted by visiting-team members in the Portal. When a member cannot submit their own evaluation because they do not have a Portal login, used a paper form, or need a mistake fixed, accreditation staff can help from the Console. This article explains how to submit an evaluation for a member, enter one by hand, correct one, or delete one. To read evaluations without changing them, see Review Evaluations on a Visit or Contact.
What This Does and When to Use It
Use Submit Evaluation when you want to follow the same steps the team member would see in the Portal, such as entering answers they gave you by phone. Use Manually Create Evaluation when you need to record details from a paper form and bypass the usual submission rules. To fix or remove an existing evaluation, use the edit or delete controls on that evaluation.
Prerequisites
- You need permission to create and edit evaluations. The buttons in this article appear only while the visit has its Portal evaluation setting switched on.
Submit an Evaluation on a Team Member's Behalf
- Open the visit and go to its Evaluations tab.
- Select Submit Evaluation.
- Choose the team member whose evaluation you are entering, then select Continue.
- Complete the same steps the member would see: choose the colleague being evaluated, fill in the form, and submit it.
Result
The evaluation is saved and appears on the visit's Evaluations tab. If the visit's evaluation process sends confirmation emails, the email goes to the team member whose evaluation you entered and is worded as though they submitted it themselves.
Warning
This path follows the same eligibility rules as the member's own submission. You can choose only the colleagues the process allows that member to evaluate, and only while the visit accepts evaluations.
A visit controls whether Portal evaluations are open and when members can submit them. When you edit a site visit, you can turn Portal evaluations on and set the open and close dates.
Manually Enter an Evaluation
Use manual entry when an evaluation came in outside Rhythm, such as on paper, and you need to record it without the usual submission rules.
- Open the visit and go to its Evaluations tab.
- Select Manually Create Evaluation.
- Choose the evaluator and the team member being evaluated, enter the date, score, and recommendation, and add any comments.
- Select Save Changes.
Result
The evaluation is saved. No confirmation email is sent for an evaluation entered this way.
No Process Rules Apply Here
Manual entry does not enforce the usual eligibility rules, the visit's evaluation switch, or the submission window, so you can pair any two team members on the visit. Two rules still apply: nobody can evaluate themselves, and only one evaluation can exist for the same pairing on the same visit.
Correct an Evaluation
- Open the visit's or the person's Evaluations tab.
- Select the pencil icon on the evaluation's row.
- Update the score, recommendation, comments, or date.
- Select Save Changes.
Result
The evaluation is updated with your changes, even if the visit no longer accepts evaluations.
You Cannot Reassign an Evaluation
The form lets you choose different people for the evaluator and the person being evaluated, but Rhythm refuses changes to either person when you save. If an evaluation is attached to the wrong pairing, delete it and enter a new one instead of trying to correct the pairing.
Delete an Evaluation
- Open the visit's Evaluations tab. The delete control is available only here, not from the person's own record. Submit Evaluation and Manually Create Evaluation are also available only from the visit's Evaluations tab, not from the person's Contact record. The Contact record's Evaluations tab is for reviewing a person's evaluation history, not for submitting, creating, or deleting an evaluation.
- Select the delete icon on the evaluation's row.
- Confirm when asked.
Result
The evaluation is removed, and the pairing becomes available for a new evaluation to be submitted.
Tip
If a member submits their evaluation in the Portal before they meant to, such as before finishing it, delete it from here. That is the recorded way to let them resubmit; there is no other way for the member to reopen an evaluation once it is in.
There Is No Undo
After you confirm, the evaluation is gone. If you only need to fix a mistake, correct it instead of deleting it.
Limitations
- Neither the on-behalf flow nor manual entry marks the resulting evaluation as staff-entered, and nothing shown to you notes that you are acting on someone else's behalf.
- Deleting an evaluation does not have its own permission; anyone who can open the visit can remove one.
